Resumo: | Introdução: A cárie dentária é uma das doenças crônica mais prevalente no mundo e a infiltração da cárie é um método terapêutico promissor quanto a sua abordagem entre ações preventivas e restauradoras no tratamento de lesões cariosas iniciais. Objetivo: Avaliar se a composição da resina afeta no volume de poros ocluídos por infiltrante em lesões cariosas naturais de esmalte. Metodologia: 51 lesões cariosas naturais proximais de esmalte, sendo lesões de mancha branca inativas sem cavitação foram selecionadas por uma examinadora calibrada para preparo das amostras através de cortes por desgaste. 1 ponto histológico na camada superficial e 4 na região central do corpo da lesão foram selecionados para quantificar volumes mineral (V1) através de níveis de cinza obtidos a partir de imagens em MicroCT, orgânico (β), de água total (α) e volume infiltrado por resina infiltrante (α2) e desfecho principal, a partir dos retardos de fase no Image J. O desfecho foi o volume infiltrado por infiltrante (αinfilt) dividido pelo percentual de água total que foi removido pela desidratação (α2/α). As amostras foram separadas em três grupos: Grupo Icon: 17 amostras infiltradas com Icon®; Grupo TB: 17 amostras infiltradas com resina infiltrante experimental com TEGDMA 75% + BisEMA 25%; Grupo TH: 17 amostras infiltradas com resina infiltrante experimental com TEGDMA 80% + HEMA 20%. Resultados: Foram analisados no Grupo Icon: 74 pontos; Grupo TB: 74 pontos; Grupo TH: 71 pontos, totalizando 219 pontos histológicos. O volume ocluído por resina infiltrante no grupo Icon® variou em aproximadamente 1 e 17%, no grupo TB em 0 a 15% e no grupo TH em 0 a 16%. Os dados de volume de água total foram fortemente associados com a razão a2inf/a2 (n de 219, coeficiente r de 0,52, IC de 95% de 0,42 a 0,61, valor de p < 0,0001, e poder maior que 99,9). O teste de Kruskal-Wallis mostrou haver efeito no desfecho (p-value = 5.445e-08; tamanho do efeito eta ao quadrado de 0.146 [IC 95% = 0.08;0.124]). Valores obtidos para mediana e 75% da diferença interquartil (DIQx0,75): ICON (n=74; mediana=0.048999; DIQx0,75=0.009306); TB (n=74; mediana=0.051085; DIQx0,75=0.007456); TH (n=71; mediana=0.056805; DIQx0,75=0.0092380). Teste entre pares de grupos não pareados – Mann Whitney foram realizados entre TH X TB (Cohen d: 0.69; IC 95%: 0.36;1.03; Valor de p: 1.121e-07); TH X ICON (Cohen d: 0.84; IC 95%: 0.50;1.18; Valor de p: 2.445e-06) e TB X ICON (Cohen d:0.24; IC 95%: -0.56;0.09; Valor de p: 0.8225) e foi observado que há efeito do tratamento, com o grupo TH tendo maior infiltração que os grupo TB (tamanho de efeito médio de 0,69) e Icon (tamanho de efeito grande de 0,84), com valores de p muito baixo e IC 95% sem incluir a hipótese nula. Conclusão: Dentro das limitações desse estudo, concluiu-se que a composição do infiltrante altera o volume ocluído por resina infiltrante e que o grupo com características hidrofílicas (TH) resultou num maior potencial de penetração quando comparado ao grupo Icon® e ao grupo com características hidrófobas (TB). |
Abstract: | Introduction: Dental caries is one of the most prevalent chronic diseases in the world and caries infiltration is a promising therapeutic method for its approach between preventive and restorative actions in the treatment of early carious lesions. Objective: To evaluate whether resin composition affects the volume of pores occluded by infiltrant in natural enamel carious lesions. Methodology: 51 natural proximal enamel carious lesions, being inactive white spot lesions without cavitation were selected by a calibrated examiner for sample preparation by wear cutting. 1 histological point in the surface layer and 4 in the central region of the lesion body were selected to quantify mineral volumes (V1) through gray levels obtained from MicroCT imaging, organic (β), total water (α) and infiltrating resin infiltrated volume (α2) and main endpoint from phase delays in Image J. The endpoint was the infiltrating resin infiltrated volume (αinfilt) divided by the percentage of total water that was removed by dehydration (α2/α). The samples were separated into three groups: Icon Group: 17 samples infiltrated with Icon®; TB Group: 17 samples infiltrated with experimental infiltrating resin with TEGDMA 75% + BisEMA 25%; TH Group: 17 samples infiltrated with experimental infiltrating resin with TEGDMA 80% + HEMA 20%. Results: Icon Group: 74 points; TB Group: 74 points; TH Group: 71 points, totaling 219 histological points. The volume occluded by infiltrating resin in the Icon® group varied by approximately 1 and 17%, in the TB group by 0 to 15%, and in the TH group by 0 to 16%. The total water volume data were strongly associated with the a2inf/a2 ratio (n of 219, r coefficient of 0.52, 95% CI of 0.42 to 0.61, p value < 0.0001, and power greater than 99.9). The Kruskal-Wallis test showed there was an effect on the outcome (p-value = 5.445e-08; eta squared effect size of 0.146 [95% CI = 0.08;0.124]). Values obtained for median and 75% interquartile difference (DIQx0.75): ICON (n=74; median=0.048999; DIQx0.75=0.009306); TB (n=74; median=0.051085; DIQx0.75=0.007456); TH (n=71; median=0.056805; DIQx0.75=0.0092380). Unpaired pairsof- groups Mann Whitney tests were performed between TH X TB (Cohen d: 0.69; 95% CI: 0.36;1.03; p value: 1.121e-07); TH X ICON (Cohen d: 0.84; 95% CI: 0.50;1.18; p value: 2.445e-06) and TB X ICON (Cohen d:0.24; 95% CI: -0.56;0. 09; p value: 0.8225) and it was observed that there is a treatment effect, with the TH group having greater infiltration than the TB (mean effect size of 0.69) and Icon (large effect size of 0.84) groups, with very low p values and 95% CI not including the null hypothesis. Conclusion: Within the limitations of this study, it was concluded that the composition of the infiltrant alters the volume occluded by infiltrating resin and that the group with hydrophilic characteristics (TH) resulted in a greater potential for penetration when compared to the Icon® group and the group with hydrophobic characteristics (TB). |
